Appointment Information

  • Who can make an appointment?
    An appointment at the Center can be made by any individual who may have symptoms of memory loss, disorientation, and confusion which may be caused by Alzheimer’s disease or a related illness. Patients may be self-referred, referred by family, private physician, or a community agency.

  • How to make an appointment
    An appointment for an assessment can be made by calling (949) 824-2382 and speaking to the Center’s staff. The staff will determine eligibility through a brief telephone interview where they obtain basic information about the patient. They can assist you with your questions and schedule your assessment. For non-English speaking patients, please notify the staff.

  • What do I need in order to make an appointment?

What you will need to have ready when you call to make an appointment are the following:

    • Patient information (name, address, and telephone number)
    • Date of birth, social security number
    • Insurance information (name, policy/group number)
    • Primary doctor information (name, address, and telephone number)

All of these pieces are necessary to complete the scheduling process.

  • What forms of payment are accepted?

Appointments can be made by calling (949) 824-2382.  Medicare, Medi-Cal, supplemental insurances, and private payment are accepted. Special financial arrangements can be made; please call the center for more information. If you are uncertain about coverage please contact your insurance provider. Special financial arrangements can be made in advance.

  • How many appointments are needed?
    An assessment at the Center typically consists of three appointments at the clinic.

  • Appointment One: Neuropsychological testing
    Approximate length of appointment: 2 to 2 1/2 hours long

  • Appointment Two: Neurological examination
    Approximate length of appointment: 1 to 1 1/2 hours long

    The first two appointments are scheduled close together often one day after another. After the neurological examination, additional tests may be ordered. As necessary, brain scans (such as Magnetic Resonance Imaging (MRI)), complete blood tests, chest x-ray, and electrocardiogram (EKG), may be performed to help diagnose treatable causes of memory loss. Arrangements for these tests at the appropriate facilities can be made either through the clinic staff or the referring physician.

    Once the neuropsychological tests have been scored and the results combined with all other tests, a family conference appointment will be scheduled.

  • Appointment Three: Family Conference
    Approximate length of appointment: 1 1/2 to 2 hours long
    Family conferences are scheduled after the assessment results have been reviewed and the patient and family members are encouraged to attend. If you have questions about the number of attendees that may attend the conference, contact the Center staff.
    A full report detailing the results of the assessment, diagnosis, and recommendations will be sent to the participant and the referring physician after completion of the assessment.

  • How do I cancel an appointment?
    Appointments at the Center are typically scheduled a few weeks in advance. Appointments can be cancelled by calling (949) 824-2382 at least two weeks in advance to avoid cancellation fees
